General Information
Yes! You can request a telehealth appointment through our patient portal, Healow, for family medicine or mental health, or Headway, specific for mental health.
We are located at 3201 South Wallace Street, Chicago IL 60616. Free parking is available on the right side of the building.
Yes, we are accepting new patients.
We offer a wide range of primary care and mental health services. Family health includes preventative care, chronic disease management, physical exams, school or work physicals, and treatment for common illnesses and minor injuries. Mental Health includes, but is not limited to Anxiety, ADHD/ADD, Depression, Bipolar, Schizophrenia, Psychotherapy, and Psychiatric Consultation.
Yes, we accept walk-ins for urgent but non-life-threatening conditions, depending on the schedule for the day. However, we recommend booking an appointment to reduce wait times and guarantee an appointment time.
Suzanne Chan speaks English and Chinese (Cantonese).
Our office is open 7 days a week! Monday - Friday from 9 AM to 6 PM and Saturday and Sunday from 9 AM to 1 PM.
Appointments
Generally, appointments are available within 1 week. Appointments can be booked through our patient portal, Healow, for family medicine or mental health, or Headway, specific for mental health. Alternatively, you can reach out to us directly for scheduling, (312) 248-8610.
To cancel or reschedule, log into your account or call us at least 48 hours before your appointment.
Please bring a photo ID, your insurance card, a list of current medications, and any relevant medical records.
An appointment confirmation email will be sent once the appointment is initially scheduled. Reminders will follow leading up to the appointment. For those coming in for mental health, in addition to a confirmation email, an invitation link to Team Headway will be sent via email.
Headway is an online platform that serves as a bridge between mental health providers and insurance companies to make therapy more accessible and affordable. For patients, Headway offers a scheduling platform and cost estimates before visits. For providers it assists with insurance credentialling, billing, and payment. Allowing providers to focus on our patients and their healthcare needs. Headway also offers assessments that help providers keep track of patients' mental health progress.
To reschedule an appointment, please contact our office directly by phone or email as soon as possible. We kindly ask for at least 48 hours' notice to avoid the $200 cancellation fee and to allow us to offer the time slot to another client.
Yes. Cancellations or rescheduling requests made with less than 48 hours’ notice may incur a $200 fee. No shows will also incur this fee. We kindly ask that you notify us as early as possible to avoid this fee and to allow us the opportunity to offer the appointment time to other clients in need of care.
Two no-shows may result in a review of your fit for continued care. Consistent no-shows can disrupt the therapeutic process and limit access for others in need. After two missed appointments without notice, we will evaluate whether our services are the right fit for you, which may result in discharge from care.
Insurance & Payments
We accept most major insurance plans that are PPOs, including Blue Cross Blue Shield, Aetna, United Healthcare, Cigna. Please call us to confirm if we accept your specific insurance provider, (312) 248-8610.
Our self-pay rate for Family Medicine related services are $250 for initial visit and $150 for follow ups. Mental Health related services are $275 for the initial visit. Follow up cost are $275 for a 60-minute appointment, $185 for a 30-minute appointment, and $150 for a 15-minute appointment.
Yes, we offer payment plans to help manage your account balance. A minimum monthly payment of $100 is required, which will be applied to the oldest outstanding balance first. A valid credit card must be kept on file, and $100 will be automatically charged each month until the balance is paid in full.
Medical Questions
Yes, we offer onsite lab services and can perform common diagnostic tests. We also coordinate with local labs for specialized testing. For patients that see us virtually, we can send lab orders to a Labcorp or Quest, for your convenience.
No, currently, we do not offer vaccinations in the clinic.
You can request a refill by calling us, using our patient portal, or asking your pharmacy to send us a request.
Miscellaneous
No, as Suzanne is licensed in Illinois, she can only see patients that are in the state of Illinois during the visit. Suzanne will not be able to see patients outside of Illinois, otherwise.
